Due to anticipated high volume, we are only taking online applications at the time. No paper applications will be accepted.

Application Process

Please make sure to complete this step as your application will not be complete without providing the requested information.
Once your application has been submitted online, it will be placed on a waiting list via a random lottery process completed by an independent third-party.
You will be notified by mail if your application has been accepted for processing. At that time, you will be asked to provide additional information such as proof of citizenship/immigration status, employment verification and others. Please make sure to respond by the deadline to ensure your application remains active.
Due to anticipated high volume of calls, we will not be able to verify your application status. All applicants who are selected via lottery for further processing will be notified by mail.
Please make sure to inform our office if your contact information (address and phone number) changes once you’ve submitted your application.
